It might be that the end of the insert has been cut off so it sits flush with the edge of the combution chamber and this is what accounts for the ragged end. It has obviously piked up bits of aluminium from the head on the way out.
If a lump like that had broken off while running you would have no piston or head left by now, there have been numerous reported problems with washers ending up in the cylinder from the inlet manifold (including me), and even something tiny makes a mess.
If you can figure out if the cut off bit was pointing downwards towards the piston when screwed in it should confirm my theory

Assuming that you can get away with not taking off the head, (I had the same thought as Neil when I saw the picture, that it had been fitted whilst the head was off and trimmed to shape) the next problem is, will the thread of the new insert be the same diameter and pitch for a replacement It may be on the limit of how much more can be cut out to accept a new thread.

liking these suggestions guys.. feeling more positive all the time. looking at the plug/insert it is a nice clean cut at the same angle all the way through.. its also covered in carbon deposits, so is not fresh. works perfectly with Neil and Ians theory..
I guess I can try the same size replacement insert with some loctite to form a nice solid bond to the hole thread. If that doesnt work, perhaps a helicoil.

You would have to fit a shorter insert though as you won't be able to trim the excess like the previous fitment.
