HELP!! HELP!! HELP!! Setting up a BW65

I have just fitted a BW65 to my car, taken it for a run and it is refusing to shift up from first to second to third. It will select all gears independently and disengage gears ok. But when in drive it will start off in first and stay there all the way to 3000 rpm. if you then ease off the accelerator it will drop into second. I have gained third once or twice by doing this. The box does whine. The most common issue is the kick down cable adjustment. The manual shows a 1.5mm gap to the crimp band but my cable does not have one. So how do I connect it up? I have attached some photos to illustrate the problem.....

Slacken the kickdown cable so it drops into 3rd on a dead flat road on the very lightest throttle between 16 and 19 mph - lock the cable off at that, then as long as you get full throttle when you floor it, kickdown will work fine and adjustment will be as near as damnit perfect.

if you don't get full throttle from the pedal and the throttle cable is properly adjusted, then the throttle pedal needs bending up to get full travel back....

The way you have it in photo 1 is correct, so if you cannot get it to slacken off enough to change up then you have an issue of low overall gearbox pressure or the throttle valve is sticky in the valve block

I am going to drop the sump at the weekend and check the insides. I have ordered fresh oil and a filter and will check the positioning of the cam whilst in there. Hopefully, the new filter, oil and cable adjustment will be enough to get it to go. Can I free up the throttle valve in the valve block whilst the box is in situ?
