I've known someone to fit an incorrect pressure cap that fitted but, wasn't long enough to seal to the bottle. Could that be it?

I had a good cap that didn't quite reach the bottom of the HT tank neck. it needed some gentle persuasion around the rim in order to make that good seal.
the giveaway was the squishy top hose indicating the cooling system wasn't pressurising but also that the leak must be above the water level as there was no water loss.

Pressure test it. Use a 1/2" bsp male to 15mm compression fitting in the rad filler cap hole. Fit a small length of 15mm pipe in it the get yourself a cheap pressure tester from a plumbers merchants that pump up with a car or bike pump. If it drops use leak detection spray also avaliable from plumbers merchant.
