Your prise it off under the small slot cut into the area of the switch, you can then gently pull it out but it has two thin cables attached. Once you have the switch out and disconnected the two wires (they just pull out gently) you can access the slotted nut that holds the gear knob on. I usual use a thin screwdriver and a mallet to tap the nut undone.
Plenty of grease will hold them in place while fitting
Yes grease.. If you take the overdrive wires out, they are very easy to feed back up the gearstick. Before you do that, check their condition. If needs replacing, then make up your own or you can buy them.
In any case its good practice to check the wires for damaged insulation or connections, I know of some that have shorted and blew the fuse.
It might just be the plastic cap/switch assembly, which can be just loose once it has snapped in to place. Mine "Fizzed" till I put a small piece of paper between the cap and the metal before fitting it.
